Non-inflatable double lumen laryngeal mask

The non-inflatable double-light laryngeal mask (DLM) or supraglottic cannula are a specialized medical device used in anesthesia and respiratory tract management. Its primary function is to provide a safe airway during various medical procedures, particularly when endotracheal intubation may not be feasible or desired.

The Wellmien ALMA-G supraglottic cannulas are an innovative and novel solution for airway management.  Soft and transparent, they are made of thermoplastic elastomer medical grade.  These supraglottic cannulas are designed to create an anatomical seal in the pharyngeal, laryngeal and perilaryngeal structures without the need for an inflatable pad, while avoiding the compression injuries that can occur with inflatable supraglottic devices.

FUNCTIONS:

Respiratory tract management:

DLM facilitates ventilation by securing the respiratory tract and allowing gases to flow into and out of the lungs.

Separate channels:

One lumen is dedicated to ventilation, separated from the second dedicated to the gastric canal. This allows you to insert a gastric tube to decompress the stomach or administer medication.  Size 1.0 consists of just one lumen.

Flexible use:

It can be used in a variety of clinical settings, including surgery, emergency medicine and critical care situations.

Less traumatic:

Compared to endotracheal intubation, the insertion of a supraglottic cannulas or DLM may be less traumatic and more comfortable for the patient.  Its position guide associated with the integrated bite piece allows a better fixation of the device and a better stabilization of the oral cavity.  The pad, anatomically shaped is made of thermoplastic elastomer material medical grade and acts as a retainer of the epiglottis, thus reducing the possibility of folding of the epiglottis and consequent airway obstruction.

ADVANTAGES:

Versatility:

The supraglottic cannulas or DLM can be used in a wide range of patients, including those with difficult airways or certain anatomical variations.

Reduced risk of trauma:

Because it does not require an inflated sleeve, there is a reduced risk of pressure-related trauma to the airways.

Gastric access:

The presence of an independent gastric canal allows for gastric decompression and administration of drugs or fluids without interfering with ventilation.

Less invasive:

Supraglottic cannulas or DLM insertion is generally less invasive than endotracheal intubation, which may be beneficial in certain clinical situations.

Overall, the non-inflatable double lumen laryngeal mask offers a valuable alternative for airway management in various clinical environments, providing a balance of advantages and disadvantages that healthcare providers should consider when selecting the right respiratory device for their patients.

Indications for use

Indicated to stabilize and maintain clear airways during routine and emergency anesthetic procedures in those interventions where patients on fasting require spontaneous ventilation or intermittent positive pressure (IPPV) ventilation. The Wellmien supraglottic cannula may also be beneficial in establishing clear airways during resuscitation of unconscious patients in pre-hospital or inpatient settings by trained and experienced staff The use of respiratory management devices and advanced cardiopulmonary resuscitation techniques.

Operating Instructions

  • Always wear gloves,
  • Certify that the packaging is closed.
  • Carefully inspect the device and make sure that the surfaces are smooth and intact and the channels clear.
  • Remove the device from its protective base.
  • Lubricate the outer side of the supraglottic cannula pad with water-soluble gel.  Make sure there is no lubricant left on the surface and much less inside the device.  Hold it like a pencil.
  • Align the patient’s axes with one hand to facilitate opening of the mouth.  Keeping the jaw open, insert the supraglottic cannula with your other hand until you find no resistance.  Check the bite line.

WARNINGS

  • Check for polytrauma.
  • Check if the patient is not wearing dentures, dental plaque or any other foreign object in the oral cavity.
  • Make sure you are using the correct size of your device.  Otherwise, the sealing pressure will be inadequate and excessive air leakage will occur.
  • Do not apply excessive force to the device during insertion.  It is not necessary to insert the fingers into the patient’s mouth during device insertion.
  • The maximum time of use of the supraglottic cannula is approximately two hours.
